Brace Adjustment

For infants with clubfoot: Adjust the affected foot to 60-70 degrees of abduction, and the normal foot to 30-40 degrees of abduction. For infants with mild deformity, if the last cast was set at 50 degrees, the shoe abduction should also be adjusted to 50 degrees.

For correction of children's pigeon-toed gait: Generally, adjust to 50 degrees of abduction for severe cases; later, it can be reduced by 10 degrees according to the recovery progress.

Abduction angle adjustment method: Keep the central screw of the disc fixed, unscrew the outer screw, rotate outward, align the scale line with the pinhole on the shoe rod, and then tighten the outer screw. (Screw holes for 0°, 20°, 40°, 60°, 80°: loosen the lower holes; for 10°, 30°, 50°, 70°: tighten the upper holes.)

Length adjustment: The length can be equal to or 4 cm longer than the child's shoulder width.
How to Wear the Brace

Put on cotton, breathable socks for the child, and cut open the toe area to expose the five toes to observe toe circulation.

Undo all straps, let the child's knee flex at 90 degrees, place the heel first, then pull the middle strap. Check through the heel openings to ensure the foot is fully fitted to the bottom, then tighten the middle strap followed by the upper and lower straps.

Insert the adjusted Denis Browne shoe rod.
Wearing Time

For infants with clubfoot who have just had the cast removed: Wear the brace continuously for 23 hours a day for the first 3 months, with a cumulative rest of 1 hour (e.g., during bathing).

After 3 months:

1st month: 20-22 hours a day.

2nd month: 18-20 hours a day.
-
3rd month: 16-18 hours a day until the child can walk independently.

Continue wearing during daytime and nighttime sleep until the child is approximately 4-5 years old. If the child does not resist, it is recommended to wear it until 6 years old to reduce the risk of recurrence.
Advice for Parents

Cast surgery for clubfoot is only the first stage of correction; the second stage is the responsibility of parents. This is a long and challenging process—parents must be mentally prepared for a long-term commitment. Strictly follow the wearing schedule; do not take chances or assume the correction is permanent and the brace is no longer needed. Research by Morcuende shows that the recurrence rate is 6% for children who wear the brace as required, compared to 80% for those who do not.

It is normal for the child to cry for 3-7 days when first wearing the brace. This is due to unfamiliarity, not pain. Do not discontinue use because of crying.

For infants aged 2-10 months who are new to the brace: During daytime free time, hold the central rod to practice knee flexion, extension, and lifting movements, encouraging the child to swing both legs together.

The brace should not be too tight or too loose. A perfectly fitting brace may become obsolete as the child grows, causing unnecessary financial burden. An overly loose brace cannot fix the foot effectively. Ensure the middle strap is snug with no gaps at the heel to avoid skin irritation from kicking and friction.
